Community Health Needs Assessment (CHNA)
Your needs are
the inspiration for our plans.
Every three years, a Community Health Needs Assessment (CHNA) report is published, outlining the primary health-related issues local residents face. Eisenhower Health is one of many organizations that uses this information to prioritize and plan additions and advancements to meet the needs identified. You can read the 2019 CHNA, and the subsequent Implementation Plan, as well as the 2022 CHNA and Implementation Plan for detail on the methodology, findings, and next steps.
