It reflects enormous dedication to your patients and their families, and your whole community should be proud. The Leapfrog Hospital Safety Grade is the only hospital ratings program exclusively focused on preventable medical errors, infections, and injuries, which claim the lives of more than 500 patients daily in the United States. The peer-reviewed, transparent grading system evaluates hospitals twice annually, in the spring and fall, based on more than 30 performance measures.
