Instructions for making a referral to the Eisenhower Diabetes Education Program
1. Eisenhower Health providers please refer internally for Pre Diabetes and Diabetes in Epic to Ambulatory Diabetes Education Ref#20. This includes Medical Nutriton Therapy with the Registered Dietitian.
These are the codes if a patient requires a Pre Authorization:
Required Codes
- G0108 at least 4 units with RN Diabetes Educator for 2 visits
- 97802 4 units for MNT Assessment with the dietitian
- 97803 at least 8 units for two MNT follow ups with RD
- G0270 Additional MNT 16 units for 4 visits
