Billing & Insurance
Eisenhower Health will ultimately be preferred providers for most insurance plans. Insurance providers make the final determination which physicians are included in their coverage. To see the plans for which we are currently preferred providers check the Covered California web site to see if your doctor of choice is included.
Billing & Insurance Policies Accepted
- We accept all PPO insurances. If we are not a preferred provider, the percentage of the bill you will be personally responsible for will be most likely higher.
- Co-pays and account balances are due at the time of service. We take cash, checks, VISA, MasterCard, Discover and American Express.
- Credit card payments can be made electronically through the Pay My Bill form. If you wish to pay by cash or check, please send all payments to our offices.
- If you have a question about billing, you can send a Customer Service message from your MyChart account. Use "Billing Question" as the subject for the message. You may also call the Billing Customer Service line at 800-453-6012. Mon - Fri, 8:30 a.m. - 4 p.m.
- Understand how your insurance works. We are preferred providers for many insurance carriers; however, it is best if you contact your carrier directly regarding your specific benefits.
Insurance Accepted for Eisenhower 365
With our Eisenhower Primary Care 365 and Eisenhower Primary Care Concierge programs, we will ultimately accept most insurance plans. The list of health plans at Eisenhower Health continues to grow. While every effort is made to ensure this list is current, please verify with your insurance before your visit.
In the event that your insurance plan has not contracted for clinic based primary care or specialty services with Eisenhower Health, you will be responsible for your bill and may receive partial payment from your insurer.
Medicare
- Medicare (traditional Medicare)
- Medicare Advantage PPO plans
- Medicare Private Fee for Service plans (PFFS)
Note: Medicare supplement plans are accepted as secondary benefits for patients with traditional Medicare.
Medicare Advantage PPO and Private Fee for Service (PFFS) plans may be out of network. Confirmation of your "In-Network" status can be made by calling your insurance.
Medicare Advantage HMO plans (e.g., SecureHorizons, MD Care Healthplan, and other Medicare Advantage HMO plans through networks such as Empire Physicians Medical Group and Desert Oasis Medical Group) cannot be accepted due to HMO requirement that HMO members must see their HMO primary care physician.

Note: Companies marked with an asterisk (*) often provide their physician networks to other insurance companies. Look for their name or logo on your insurance card.
HMO Plans (Health Maintenance Organization Plans)
At this time, Eisenhower 365 cannot accept HMO programs such as California Care, SCAN Healthplan or other Medicare Advantage or commercial HMO plans, such as Desert Oasis Medical Group, due to HMO requirements that HMO members must see their HMO primary care physician.
