Eisenhower Health System Privacy Code
Implementation July 1, 2024
With the rise in scam technology and complexity, Eisenhower Health is taking extra precautions to protect patient medical information with the implementation of a four-digit privacy code.
Beginning July 1, anyone requesting medical information over the phone will be required to provide a privacy code — including the patient, parents of children who are patients, family, friends, or a designated representative. It is also required when medical information is being requested in person and the patient is unavailable.
Getting started
In June, all Eisenhower Health patients will receive a letter with their privacy code. Codes may be changed, if desired, starting July 1. If the patient, the parent of a child who is a patient, or Medical Durable Power of Attorney for the patient has forgotten their privacy code, the Health Information Management department will take steps to verify identity and provide the code. If an individual other than those listed above has forgotten the patient’s code, they will be asked to contact the patient for the code. They will not be provided any medical information without the code.
How do I change my code?
At your next visit when checking in, please make the request to change your code and we will be happy to assist you.
Protect your privacy code
Eisenhower Health will use your four-digit privacy code to communicate with you, your friends or family, or designated representative. It is your responsibility to protect it and only share this code with those people you wish to receive protected health information about your condition.
Does the privacy code authorize release of hard copies of my medical records?
The answer is no. Any request for individuals to receive hard copies of your medical record will still require you to request records from the Health Information Management department or obtain online here.
